What does that mean?
You can get coverage through FBN Health without completing any health statements at a set rate. If you’d like to be considered for an even lower rate, simply complete the full application and corresponding health questionnaire.
What do I need to do next?
- Download and review the rate sheet
- Select your plan and complete the top section
- Once you have completed, email the form (scan, photo or pdf) to fbnhelp@mkeithins.com with subject line 40 Square Co-Op. You will receive a response within 24 hours.
